Gnathology Rome - Diagnosis and treatment for bruxism and temporomandibular disorders

A professional approach for the well-being of your masticatory system

Our masticatory system is an extraordinarily complex mechanism: it regulates chewing, phonation, and swallowing, thanks to the interaction between teeth, muscles, temporomandibular joints (TMJ), ligaments, and sophisticated neuromuscular control.

When something disrupts this delicate balance — as happens in bruxism or temporomandibular disorders (TMD) — uncomfortable and debilitating symptoms can arise: muscle pain, joint clicking, difficulty in jaw movements, headaches, or even neck pain.

At our dental practice in central Rome, located at Piazza Casalmaggiore, 3 – 00187 Rome, we address every case of bruxism or TMJ dysfunction with expertise and empathy, helping each patient regain comfort and peace of mind.

What is bruxism and what symptoms can it cause?

The bruxism is an involuntary parafunctional activity that manifests through clenching or grinding of teeth. It is classified as:

  • Nocturnal bruxism (grinding/clenching during sleep)
  • Daytime bruxism (clenching, related to stress or concentration)

The most common symptoms include:

  • Worn teeth with visible surface wear
  • Muscle pain or stiffness upon waking
  • Bruxism and muscle tension headache
  • Bruxism and neck pain
  • Bruxism and headache
  • Sensation of dizziness or muffled ear
  • Joint clicking or sounds
  • Masseter muscle hypertrophy

The correlation between bruxism symptoms and pain isn’t always linear: in many cases, the patient is asymptomatic, while others develop significant disorders requiring dedicated management.

What are the causes of bruxism?

Bruxism has multiple contributing factors:

  • Emotional stress: among the most common causes
  • Genetic factors and predisposition
  • Consumption of certain substances (caffeine, nicotine, alcohol, medications such as SSRIs)
  • Central nervous system disorders
  • Occlusal factors (now considered less determinant)

Childhood Bruxism

It is very common among 20-38% of young patients, tends to be self-limiting, and in most cases does not evolve into chronic bruxism in adulthood.

Temporomandibular disorders and TMJ dysfunction

The temporomandibular disorders (TMD) encompass a group of dysfunctions involving the temporomandibular joint and related muscles.

The temporomandibular joint is one of the most complex in the human body: it allows rotation and translation movements, thanks to the presence of an articular disc and a sophisticated muscular and ligamentous system.

An alteration in its function can cause:

  • Pain during jaw movements
  • Joint clicking or crepitus
  • Limited mouth opening
  • Jaw deviations
  • Facial and neck pain
  • Balance disorders

How is the diagnosis made?

At Studio Dentistico Alessio Triestino, diagnosis is based on a rigorous and personalized clinical protocol:

  1. Detailed medical history to assess duration, intensity, and characteristics of symptoms
  2. Muscle examination: evaluation of masseters, temporalis, pterygoid, and cervical muscles
  3. TMJ examination: static and dynamic palpation, analysis of joint sounds and functionality
  4. Occlusal and dental examination
  5. Postural assessment and related areas
  6. Advanced imaging (CBCT, MRI) when necessary

Each phase aims to identify the predominant causes and personalize therapy.

Conservative, reversible, and customized treatment approach

Trattamenti for bruxism and TMJ disorders follow principles of conservativeness and personalization:

  1. Education and counseling about the problem
  2. Relaxation and stress management techniques
  3. Mandibular and myofunctional physiotherapy (if indicated)
  4. Treatment with custom dental bite guard in Rome: the muscle relaxation bite guard, digitally designed in rigid resin, is the most commonly used device at our practice.

It allows to:

  • Stabilize occlusion
  • Reduce parafunctional activity
  • Protect dental surfaces
  • Reduce muscle and joint pain

In cases of bruxism – psychological causes – complementary psychological support may be useful.

FAQ

1. What is bruxism and how does it manifest, both day and night?

Bruxism is an involuntary activity of the masticatory muscles that causes teeth clenching or grinding. It can occur during sleep (nocturnal bruxism) or while awake (daytime or stress bruxism). During the day, it’s often associated with moments of concentration or emotional tension; at night, it occurs unconsciously during certain sleep phases.

Symptoms include dental wear, facial muscle pain, jaw stiffness upon waking, muscle tension headaches, neck pain, tinnitus, and in some cases dizziness. The TMJ can be involved with clicking, joint pain, and limitations in jaw movements.

Bruxism has multiple causes: emotional stress, genetic predisposition, substance use (nicotine, caffeine, SSRIs), sleep disorders, and, less frequently, occlusal factors. In children, it’s often self-limiting.

Diagnosis is based on thorough medical history, muscle and joint examination, occlusal evaluation, and TMJ imaging (CBCT or MRI, if necessary). In complex cases, polysomnography may be useful.

Treatment is conservative: counseling and stress management, mandibular physiotherapy, and, when indicated, custom-made splint therapy. For more complex disorders, treatment can be integrated with sleep specialists or psychological support.

The splint stabilizes the joint, reduces parafunctional activity, protects teeth, and helps relax muscles, relieving muscle and joint pain. It’s a reversible and personalized treatment.

At Studio Dentistico Alessio Triestino on via del Tritone, Central Rome, you will find a comprehensive and multidisciplinary approach to treating bruxism, with the possibility of integrating dedicated dental and physiotherapy trattamenti.
